Questionnaires

To begin the process we need to gather key information for preparing your return.  The following link will provide you a downloadable MS Word doc that can be attached to an email and sent to me, or you can mail the form back to me. Please note at the top of the questionnaire the year is left blank. Fill-in the year you are submitting.  The PDF Version of the questionnaire requires that you install Acrobat Reader 9. Once you do this, you can save the form  to your computer and call it back up as you fill in the information. If you don't have Acrobat Reader 9 it is a free download from www.adobe.com.
